Preparing For Your Appointment

  • Coming in for an office visit? Remember to bring:

    • List of all current medications, including dosage and frequency, including over-the-counter and herbal supplements

    • Insurance card(s), co-pay and picture ID

    • Copies of recent tests, imaging reports (x-rays, CAT Scans), past procedure reports or anything you want Dr. Reich to review during your appointment

  • Having a procedure? Remember to:

    • Review your preparation instructions

    • Wear loose comfortable clothing

    • Bring current medication and allergy list

    • Bring a companion who can escort you home following your procedure

  • We accept most major insurance plans. See a complete list of accepted insurance plans below, or call the office to make sure your insurance is accepted.

    It is your responsibility as our patient to provide accurate and current information at the time of service so that we may bill your insurance.

    Co-pays, co-insurance, deductibles, deposits and past due balances are payable when you visit our office.

    Several local bus lines serve the Forest Hills area. If you’re traveling by bus, we recommend using Google Maps and entering our address for the best route:

    By Car

    • From Grand Central Parkway:
      Take the exit toward Jewel Avenue / Forest Hills, then follow local streets toward Queens Boulevard.

    • From the Long Island Expressway (LIE):
      Take the exit toward Woodhaven Blvd / Queens Blvd, then follow signs toward Forest Hills.

    • From Queens Boulevard:
      Continue along Queens Boulevard to 112-03 Queens Boulevard.

    Parking

    Street parking is available in the area but may be limited during peak hours. Please allow extra time to find parking.
